MANILA, Philippines --- The Chinese vessel failed to initiate measures to prevent the risk of collision and eventual sinking of an anchored Philippine fishing boat in Recto Bank (Reed Bank) in the West Philippine Sea on June 9, the official joint investigation report of the Philippine Coast Guard (PCG) and Maritime Industry Authority (Marina) concluded.
